Apotemnophilia: Arousal To Oneself As An Amputee
Not to be confused with acrotomophilia – a general attraction to amputees – apotemnophilia has a more personal edge, and a potentially dangerous one. Apotemnophilia is about being attracted to the idea of yourself as an amputee. While that may sound like a wild fantasy, some people go the extra mile. They've been known to remove their own limbs to make this fantasy a reality. One man (who had his name changed) told The Atlantic, "Well, it was definitely like an obsession. Until I cut my leg off, of course... It was kind of messy. I did it with a log splitter.” A log splitter.
As you can imagine, it’s difficult to get a doctor to perform what many view as a totally gratuitous procedure, and at-home amputations can be painful and deadly.
Sadomasochism: Arousal From Giving Or Receiving Pain
It may be really well known, but arousal from giving (or receiving), physical and psychological pain is about as textbook painful as it gets. Sadomasochism (or S&M) is all about pain, and many folks switch between giving and receiving, though some have strict roles. But when you’re dealing with whipping, spanking, cutting, you need to have some strong safe words in case things get really dangerous.
It's also really common. What's the draw? Well, one man explained to Cosmopolitan:

Probably made most famous by Issei Sagawa, who killed and ate a classmate in France and then walked free, this phenomenon is an arousal to eating another person’s body parts. Sagawa told Vice about devouring his victim, explaining,
“The first thing I did was cut into her buttock. No matter how deep I cut, all I saw was the fat beneath the skin. It looked like corn, and it took awhile to actually reach the red meat. The moment I saw the meat, I tore a chunk off with my fingers and threw it into my mouth. It was truly a historical moment for me.”
Though it’s obviously a lot more painful (and straight-up fatal) for the victim, the violence of the fetish and considerable jail time involved earns it a place on the list. Not everyone will be as "lucky" as Sagawa, of course, and will probably wind up in prison.
Chremastistophilia: Arousal To Being Robbed
You may dread the idea of being robbed or mugged late at night, but for those interested in chremastistophilia, it’s the dream. There are people out there who get turned on by being held up: in the robbery sense, that is. Though it might not involve actual physical pain, it definitely revolves around the fear of it (and maybe some actual pain). Kris Saknussemm, a sex researcher, explained the phenomenon:
“I’ve met several chremastistophiles, all of whom had been arrested on petty charges at some point in their lives — drug possession, minor theft, etc. All expressed a strong libido, but also a climax dysfunction. They got aroused, they just didn’t get off easily.
What magical thing finally provided that long-awaited release? The experience of being taken advantage of — which is different from out-and-out assault. It’s a variation on biastophilia, the perverse attraction to being raped, but the key distinction seems to lie in the impending threat itself. ‘Give me your wallet and nobody gets hurt’ — that kind of thing.
One British gentleman proudly displayed the scar he received from a knife wound in the course of a mugging — an event which he said led to a spontaneous ejaculation, the most powerful and substantial he’d ever experienced.”
