In Shutter Island, every time Teddy Daniels smokes in the movie he gets his cigarettes lit by someone else which shows he was a patient because mental patients are not allowed to have matches.
In Black Swan, when Nina is watching Lily dance for the first time, Leroy enters from behind and begins to talk to her. However, Leroy is never shown walking through the door in the shot, and instead seems to materialize behind Nina out of thin air, hinting it may be a hallucination.
In Se7en, when Mills and Somerset are raiding the murderers apartment, they find photos of the 7 victims. Mills finds the photo of himself from the staircase, hinting that he is one of the 7 sinners in the story.
In Children of Men, Theo takes Kee and Miriam to Jasper’s house, thinking it to be a hidden safe house, but The Fishes find it anyway. Earlier in the film, one of The Fishes is seen behind Theo on the train he is riding to visit Jasper, foreshadowing how they knew about Jasper’s house.
In The Silence of the Lambs, Clarice fails an FBI drill by forgetting to look over her left shoulder when entering a room. Later, she misses a hint to her suspect's identity by making the same mistake. The detail that Clarice misses is a painting of a moth/butterfly. The killer that she's chasing throughout the movie puts a cocoon inside the mouth of every victim that he kills. At this point in the movie, she does not know if this is the suspect's house, and the painting would have been a hint to her.
