My Hero Academia can be pretty brutal when it comes to battle outcomes. Not only do many characters lose their lives, those who do survive are often critically injured. Characters develop permanent scars, and lose organs, limbs, and other body parts. Even in a world where healing quirks exist and are readily available, some of these injuries are so severe that they can't be healed.
Both heroes and villains are subject to the pain. All For One and All Might were both severely hurt by their battle, with All For One losing his sight and requiring permanent life support, and All Might losing his stomach and damaging his lungs. Meanwhile, both Re-Destro and Aizawa have to cut off their own legs to avoid even worse damage.
Which of these injuries would you least want to experience?
Being forced to cut off your own leg sounds like one of the most horrific things a person can endure, but Aizawa does just that when he gets hit in the leg by a Quirk-Destroying bullet. Not wanting the drug to spread through his system and destroy his quirk, he quickly decides to sever his own leg.
Not long after that, he loses yet another body part - his eye. Shigaraki grabs him by the face and ends up destroying his eye using Decay.

All Might Loses His Stomach After Fighting All For One
Though All Might managed to defeat All for One, he didn't come out of the fight unscathed. Rather, he came out with injuries so serious that they'd eventually end his career. His various broken bones are the least of his problems - his stomach was completely destroyed, and his respiratory system was so badly damaged that he still coughs up blood years later.
His ability to hold his muscle form decreases greatly, and eventually disappears altogether. Once this happens, he has no choice but to retire from his position as a Hero.
During his showdown with Dabi, Hawks sustains major burns to his back, which nearly obliterate his wings. Dabi's quirk produces blue flames, which means that the flames heat up to a minimum of 4532 degrees Fahrenheit or 2500 degrees Celsius.
The fact that he survived a burn like that is astonishing, and the fact that he passed out is totally understandable. Luckily for him, Tokoyami was there to carry him to safety.
While fighting a group of High-End Nomu, Mirko tries to defeat them using her ultimate move, Luna Fall. But that doesn't work out, because one of the Nomu, Robot, rips off her arm with its Spatial Distortion Quirk. Amazingly, she's able to keep fighting for a while by using her hair to stem the bleeding. Not only that, but she sustains a gigantic wound in her side which likely impacted her organs.
But in the end, she's too badly injured, so she's rescued by Endeavor and taken to a hospital.
During the Kamino Ward Incident, Best Jeanist takes a massive hit from All For One. As a result, he loses a lung.
While it would have been totally understandable for him to give up on all things Hero after that, he ends up working with Hawks to help him go undercover with the League of Villains. Specifically, he goes into cryogenic stasis so that Hawks can pretend that he annihilated him as proof of his loyalty to the League.
Dabi - also known by his original identity, Toya Todoroki - has an extremely powerful fire quirk. But there's one big problem. His body struggles to handle his fire quirk, so as a child he often burned himself. His father discouraged him from continuing to practice with it, but because his father made it quite clear that he couldn't care less about him if he didn't live up to his original expectations, there was no way that he was going to stop.
When Toya hit puberty, his flames turned blue - making them much hotter than before. What's more, he learned that his flames got bigger and hotter when he was worked up. He wanted to show his father his new abilities, but because Endeavor didn't approve of his self-training, he never showed up. Toya's ensuing emotional meltdown caused a wildfire that not only devastated the area, but supposedly incinerated Toya, leaving only a bit of his jawbone behind.
Toya didn't pass away - his charred body was taken by the League of Villains. He spent several years in a coma before eventually recovering enough to function. He suffered from extensive scarring, his tear ducts were destroyed, and he had decreased sensitivity to physical pain because of damage to his nerve endings.
